Our Lady Queen of Heaven journeys with families in this vital sacrament that begins our initiation into Christ. Below is a description of the process to have your infants and young children baptized. For children ages 7 and older and for adults, see “RCIA/RCIA for Children” for more information, or call us at 337-477-1236.

Steps for Godparents preparing for Baptism at Our Lady Queen of Heaven Catholic Church:
Baptismal Preparation Course Information
All parents who have not previously taken the Baptism Preparation Course (within the last 10 years) must do so before the baptism of their child.
The Formal Baptismal Preparation Class has been canceled for the time being. The Baptismal Preparation Course is now offered on an individual basis. Come by the church office to pick up the course booklet after registering online for a Baptism. Once the booklet has been read, call for Deacon Nixon at the office, and a discussion with Deacon about the material will complete the course.
Required Forms and Documents
All required forms should be submitted to the church office before a Baptism may be scheduled.
These forms include the Baptismal Registration Form, a current Sacramental Record (no more than six months old from church of Baptism) for each Catholic parent (call the church of your Baptism with your full name and birth date to receive this); a Certificate of Marriage from the Catholic Church of parents marriage; a Birth Certificate for the child to be baptized; a Godparent Form for each Catholic Godparent, and proof of Baptism Preparation Class for Parents and Godparents.
A copy of all documents must be on file in the church office before the Baptism can be scheduled. You may email them to [email protected] or you may bring them to the church office for copies to be made. Once all your forms are completed and submitted, the file will be handed over to be approved by the clergy for scheduling. You will then be notified to choose a celebrant for the baptism and a variety of dates on which your family is available.
Baptisms of children under the age of seven ordinarily occur on an individually scheduled basis. Please enter the church through the North Entrance (the Prien Lake side of Lake Street) about 15 minutes before the appointed time. You may have any number of people at the Baptism you desire as instructed by the celebrant. Again, scheduling for a Baptism will only be done once all registration forms and required documents are submitted and approved.
There are no Baptisms during Lent.
If your child is 7 years of age or over, he/she must enter the OCIC program through Robin Suire, Coordinator of PREP. Click here to see the OCIA for Children Page.
