Confirmation Registration Instructions

There are four different forms you must complete to register for Confirmation. They are listed in the links on the right hand column of this page. They are Confirmation Registration, Saint and Sponsor Form, Confirmation Retreat Registration Information & Form and Code of Conduct. If you prefer to fill out your forms by hand instead of on the computer, you may print all of these forms by clicking on the Confirmation Registration Package Link on the right hand column.

- Payment must be made to complete registration. Clicking the submit button will bring you to the payment page. Once you have entered and submitted your payment information please return to the Confirmation Registration Page. On the right side, click and complete the Saint and Sponsor Form, the Confirmation Retreat Registration Form and Code of Conduct. Your registration will be submitted to the PREP office. If you are not making payment online, please use the printable registration form available in the Confirmation Registration Information Package Link to the right and return by mail or in person to the PREP office.
