GODPARENT REGISTRATION INFORMATION

Dear chosen Godparent,
Thank you for accepting the great responsibility to be a part of a child’s journey of faith. This is a great honor that carries with it serious responsibility. Church Law requires that both parents and Godparents “are to be instructed properly on the meaning of this sacrament and the obligations attached to it” (Canon 851). Therefore, you will be required to attend a Baptism Preparation Class if you have not already done so for a previous godchild. (To register for the class, call 477-1236 or go to https://olqh.org/baptism.) In addition, please carefully read the following description of the role of Catholic Godparents and the requirements. Afterwards, please fill out the registration form, have it signed by your pastor of the church in which you are registered and return the signed form to the church office.
The following information is taken from the General Instruction to the Rite of Baptism for Children and the Roman Catholic Church’s Code of Canon Law:
“In the baptism of children…Godparents are to be present in order to represent both the expanded spiritual family of the one to be baptised and the role of the Church as a Mother. As occasion offers, godparents help the parents so that children will come to profess the faith and live up to it.
In the actual celebration of baptism, the part of the Godparents is to…together with the parents, profess the Church’s faith, in which the children are baptized.
Therefore Godparents, chosen by…the families of children to be baptized, must, in the judgment of the pastor, be qualified to carry out” the above responsibilities.
Requirements for Godparents:

1. There is to be only one male godparent or one female godparent or one of each.
2. Those designated must have the capability and intention of carrying out the responsibility of a godparent and be mature enough to do so. A person sixteen years of age is presumed to have the requisite maturity.
3. Those designated as godparents must have received the three Sacraments of Initiation of the Roman Catholic Church, Baptism, Confirmation, and Eucharist (and must submit this information in the form of a sacramental record less than six months old to THEIR parish priest along with the godparent form to be signed, and the FORM returned to OLQH), and be living a life consistent with the Catholic Faith and in the state of grace.
4. Those designated as godparents must be practicing members of a Catholic Church.           At the request of the parents, a baptized and believing Christian NOT belonging to the Catholic Church and never Baptized in the Catholic Church, may act as a Christian witness along with a Catholic godparent, where there is only one Godparent. A witness must be Baptized in the name of the Father, Son and Holy Spirit and submit a Baptism certificate to the office.
